Balloon Mitral Valvotomy

Balloon mitral valvotomy is a catheter-based treatment for suitable rheumatic mitral stenosis, restoring valve opening without open-heart surgery.

Procedure Overview

What Is Balloon Mitral Valvotomy?

A catheter-based treatment for selected rheumatic mitral stenosis, especially when symptoms are progressive.

Procedure Overview

Balloon mitral valvotomy (BMV) uses a specialised balloon catheter passed through the femoral vein and the interatrial septum to split the fused mitral valve commissures. The aim is to increase the valve opening and improve forward blood flow, reducing pulmonary congestion and improving exercise tolerance without the trauma of open-heart surgery.

Who Is Eligible?

Suitable Cases for Balloon Mitral Valvotomy

This procedure is most appropriate for a specific subset of rheumatic mitral stenosis patients.

Clinical ScenarioTypical Suitability
Severe rheumatic mitral stenosis with symptomsSuitable when anatomy favours commissural splitting
Mitral valve area <1.5 cm² and limiting breathlessnessOften a strong indication
No heavy calcification or thrombus in the left atriumImportant selection criterion
Patients with AF but stable anatomyConsidered after careful rhythm and anticoagulation review
Procedure Steps

How the Procedure Is Performed

The technique is planned around echo measurements and access considerations.

1

Evaluation

A transthoracic and transesophageal echo assesses valve anatomy, left atrial size, and general suitability for the balloon technique.

2

Access

A catheter is introduced through the femoral vein, and the valve is crossed under fluoroscopic and echo guidance.

3

Balloon Dilatation

A balloon is inflated across the mitral valve to split the fused commissures and enlarge the valve opening.

4

Recovery

Patients are observed for complications such as MR, embolisation, or tamponade and usually recover rapidly with close monitoring.

Benefits & Risks

Benefits and Key Risks

The primary benefit is symptom relief without a sternotomy, but careful patient selection is essential.

Core Clinical Benefits

  • Improves forward flow and reduces breathlessness in suitable patients
  • Avoids open-heart surgery in selected rheumatic stenosis cases
  • Often allows faster recovery and shorter hospital stay

Potential Risks & Incidence

  • Mitral regurgitation: A small risk of acute worsening leakage if the valve tears or splits unevenly.
  • Embolic events: Left atrial thrombus or debris can rarely embolise if not carefully screened beforehand.
  • Pericardial tamponade: Rare but serious complication requiring urgent management.
Recovery

Recovery After Balloon Valvotomy

Most patients recover quickly when the procedure is done in a suitable candidate.

FAQs

Common Questions

Key questions around balloon mitral valvotomy and valve repair choices.

01Is balloon mitral valvotomy the same as open-heart surgery?

No. It is a catheter-based procedure done through the groin and vein, without a sternotomy. It is appropriate only for selected patients with suitable anatomy.

02When is surgery preferred instead?

Surgery may be preferred when the valve anatomy is not suitable for balloon splitting, when there is significant regurgitation, or when there are other structural reasons for surgical repair or replacement.
